Dr. Joe Darrow (Costner) is head of emergency services for Chicago Memorial Hospital and a trauma expert. When his wife Emily (Thompson) dies while on a medical mercy mission in Venezuela, he takes little comfort in his professional knowledge.
Surrounded by items that remind him of her, including images of dragonflies, Darrow tries to move on. Fulfilling a promise made before his wife's death, Joe visits Emily's former patients. Through the young patients' near-death experiences, Joe begins to wonder if his wife is trying to communicate with him from beyond the grave.
